Amy Suzanne Pohlman, 48, was charged for a faked license and for making up her history. She even commissioned a plaque for an award that does not exist. The accused lives in Jacksonville and has been arrested for two counts of practicing as a Health Care Professional without an active, valid Florida license. She has been booked into the county jail on Monday with a $50,000 bond.

The St. Johns Couty Sherrif's Office said on their report on Monday that Pohlman was hired as a Nurse Administrator for Ponte Vedra Home Care in October 2015. She had documents that showed that she had a license at the time.

She then claimed to have a PhD and was an Advanced Registered Nurse Practitioner. As part of her duties she did writing up care plans, client visits and even assisted with the company's marketing.

During her employment she became familiar with more clients. She branched out and offered nursing care independently and provided services as a Care Manager.

In June, the company opted to re validate her license and couldn't find any record. The Florida Health Department was notified and Pohlman resigned during the investigation. The case was investigated with the Jacksonville Sheriff's Office and Florida Attorney General's Medicaid Fraud Control Unit in September according to St. Augustine.

Detectives said they discovered that Pohlman signed a DNR order, created a fake ARNP license and did not hold a degree in nursing. She claimed to have won an award from Trustees of the Mayo Clinic: Alzheimer's Disease Research Center, issued on March 9, 2016, in St. Paul, Minnesota, which doesn't exist. She paid for the plaque to be made according to WOKV.

She was also charged with performing a physical exam on an applicant of the home care company. More charges are pending further investigation.
